Care on the way home

Care Connect has been delivering Compacks since it was piloted in 2003 and has grown to be the biggest provider in the state. With the latest contract, Care Connect expanded service delivery into regional NSW, as well as metropolitan local health districts. With offices located in Regent Street, Care Connect employs a team of 23 people.

Compacks provides community services for up to six weeks to people returning home from a participating NSW public hospital. It is a transitional care program with services commencing once an individual has been discharged from hospital.

From the Redfern office, Care Connect’s specialised Compacks Team comprises 15 dedicated and hard-working staff. The Team delivers Compacks across five local health districts, working with more than 20 hospitals and supporting 3,000 people annually to safely return home from hospital.

Compacks is a highly regarded service, an effective rapid-response service, as evidenced by recent client feedback:

“The service I received was excellent and all the staff that had contact with me couldn’t do enough for me. The care person who took me shopping, cleaned and did one doctor’s visit with me was kind, hardworking and compassionate. I wouldn’t have coped well if I didn’t have this service for the six weeks I did.”

“The gracious, competent and friendly support of the carers was very much appreciated.”

“Care Connect provided excellent service for my mother after discharging from RNS. They assisted in getting home care for my mother immediately after six weeks with BCS, even though there was a long waiting list as my mother has severe dementia and lives at home.”

“The women who assisted my father after his stay in hospital were caring, thorough, friendly … our case manager was accessible, friendly and highly professional …”

“Many thanks for all your wonderful help, I really do appreciate it.”
