Care Connect has been delivering Compacks since it was piloted in 2003 and has grown to be the biggest provider in the state. With the latest contract, Care Connect expanded service delivery into regional NSW, as well as metropolitan local health districts. With offices located in Regent Street, Care Connect employs a team of 23 people.
Compacks provides community services for up to six weeks to people returning home from a participating NSW public hospital. It is a transitional care program with services commencing once an individual has been discharged from hospital.
From the Redfern office, Care Connect’s specialised Compacks Team comprises 15 dedicated and hard-working staff. The Team delivers Compacks across five local health districts, working with more than 20 hospitals and supporting 3,000 people annually to safely return home from hospital.
